If no selector is specified, all blocks will be processed as element blocks
unless the input was generated from IOSS, which aids in identifying
element/sideset/nodeset blocks.

Also enable vtkIOSSReader to read the original block ids.

Also fix some variable naming conventions for consistency.

Avoid using maps and use an array counter instead.

This is needed so that we can easily extract the correct
block id, if the dataset was created from vtkIOSSReader

For some reason this fixes an issue where with more than 1 core
all timesteps are traversed twice.

CopyAllocate should be used to be able to copy id based arrays.
