- Find/replace of KWSYS_NULLPTR with nullptr
- Find/replace of NULL with nullptr
- Manually fixed -Wzero-as-null-pointer-constant warnings
- Careful review to fix some comments, string output, etc.

This converts the path to a format suitable for passing to Windows APIs.

Windows was reporting errors in `wchar_t` which was not reinterpreted as
UTF-8 which is expected from kwsys errors. This basically made error
messages appear as truncated since the UCS-2 byte array had embedded
byte NUL characters.

new style defined by `.clang-format`.  Use `clang-format` version 6.0.

Rather than just testing for explicit platforms in the dynamic loader,
i.e. old BlueGene and Cray systems, leverage CMake's platform knowlege
to determine if shared libraries are supported.

Per-source copyright/license notice headers that spell out copyright holder
names and years are hard to maintain and often out-of-date or plain wrong.
Precise contributor information is already maintained automatically by the
version control tool.  Ultimately it is the receiver of a file who is
responsible for determining its licensing status, and per-source notices are
merely a convenience.  Therefore it is simpler and more accurate for
each source to have a generic notice of the license name and references to
more detailed information on copyright holders and full license terms.

Our `Copyright.txt` file now contains a list of Contributors whose names
appeared source-level copyright notices.  It also references version control
history for more precise information.  Therefore we no longer need to spell
out the list of Contributors in each source file notice.

Replace KWSys per-source copyright/license notice headers with a short
description of the license and links to `Copyright.txt` and online
information available from "https://cmake.org/licensing#kwsys".

Run the `filter-notices.bash` script to perform the replacements mechanically.
Manually fix up the shebang line in one file.

The clang-format tool sorts `#include` lines within contiguous blocks.
Separate blocks with blank lines and sort the include lines within them
so that clang-format does not re-order anything.

Drop "kwsys/stl/*" and "kwsys_stl::" and use the standard headers and
namespace instead.  Drop the now-unused KWSYS_STL_HAVE_STD check.

The DynamicLoader::LibPrefix and DynamicLoader::LibExtension methods
previously hard-coded the module name components for each platform.  Set
them from the CMAKE_SHARED_MODULE_PREFIX and CMAKE_SHARED_MODULE_SUFFIX
CMake variables instead.  This ensures consistency in a program that
uses these methods to construct the file names for its own modules.

This converts the KWSys license to a pure 3-clause OSI-approved BSD
License.  We drop the previous license clause requiring modified
versions to be plainly marked.  We also update the KWSys copyright to
cover the full development time range.

BUG:  FormatMessage can return a NULL message.  Add check for NULL pointer.

COMP: also build the static dummy loader on Cray Catamount

BUG: revert yesterday patch. The implementation was correct. The problem was that _WIN32 was forced to be #define on cygwin when included from ITK, which was miss matching the implementation from the declaration. Put extra condition for CYGWIN system
