Consolidate markdown files into docs
This moves documents historically organized under the Documentation/Doxygen and Documentation/dev directories into the Documentation/docs one.
It also simplifies the generation and maintenance of the documentation by removing obsolete manual substitutions from vtk_documentation.py.
This will have to be rebased and updated after integrating the following merge requests:
