Documentation cleanups

Christos Tsolakis requested to merge christos.tsolakis/vtk:docs-prepare into master

Use a single top header per readme file which is used to generate contents in TODO. The rest of the clean up is related to TODO.

This also brings https://vtk.org/Wiki/VTK_Coding_Standards in c6db5b86 inside this repository . The CI/testing sections of this document need to be updated.
