ThreadIndicesExtrude fix

I'm adding an ADIS reader to VTK, which uses CellSetExtrude and uses the CleanGrid filter to convert from VTK-m DataSets to VTK DataSets. Adding CellSetExtrude to DefaultTypesVTK.h uncovered missing GetThreadIndex() in the classes in ThreadIndicesExtrude.
