I made a couple of changes to the DDCMD reader. I corrected a bug
where in the case of a 2d mesh it set the z dimension to 2 instead
of 1. I enhanced the reader so that it would provide rectilinear
domain boundaries so that ghost zones would be automatically added
when necessary.
