pqPipelineModel: ensure extractors are set up in constructor

Cory Quammen requested to merge cory.quammen/paraview:populate-with-extractors into master

When creating a pqPipelineBrowser, any extractors in the pqServer were not being added, hence they would not show up in the Pipeline Browser. Fixed that.
