use same color map object whether surface or volume rendering

In ParaView 2, the scalar bar does not display the colors in the lookup table used for volume rendering. This is because surface rendering uses vtkPVColorMap, and volume rendering uses vtkPVLookupTable. Please don't follow this approach in ParaView 3; both surface and volume rendering should use the same type of lookup table / color map object so that the scalar bar displays correct scalar -> color mappings regardless of whether you're doing surface or volume rendering.
