hdf5 visualizing mesh problems

I am trying to visualize FEM data in Paraview from a hdf5 file which I have created. The reader I am using is VizSchema files. I get the following problems. (see attached). I get holes in the mesh which should not be there. Seems to be some problems with the connectivity of the elements. As seen in the attached picture the view is set to surface with edges. If I try to do just one element it works fine. But when I do the complete model it fails. The model is just a simple beam for testing.
