In vtkVolumeOfRevolutionFilter, fix assignments to output cell locations.

This MR extends TestVolumeOfRevolutionFilter to expose a bug in the assignment of cell locations, and patches vtkVolumeOfRevolutionFilter to assign the correct cell locations.
