Fixing xml unstructured grid reader to properly read in polyhedra.

This is mantis issue http://www.paraview.org/Bug/view.php?id=14069 where if some pieces have polyhedra in them and others don't then the polyhedra face information in the unstructured grid doesn't get set properly.
