Access list of outputs from the PulseEngine or CDM instead of hardcoded

Replace: https://gitlab.kitware.com/physiology/PulseUnityAsset/blob/master/Scripts/PulseEngineDriver.cs#L26-41 by something dynamic that is generated by the PulseEngine, based on: https://gitlab.kitware.com/physiology/engine/blob/2.x/engine/c/PulseEngineC.cpp#L237
