Improved respiratory system and support for new disease states

Put dyspnea back in asthma attack death to cause irreversible state.  This will eventually be removed with the respiratory fatigue model.

Added damping to pleural cavity venous effects (mainly from pneumothorax) and tweaked bilateral pneumothorax scenario timing to prevent error from heavily diseased patient.

Added outputs to pneumothorax scenarios.

Update scenarios with PulmonaryResistance data request.
Tweaks to respirtory system validation data.

Updated respiratory validation spreadsheet.
