C++ HowTo Updates

Update Respiratory Driver Waveform

Mirror the patient conditions found in the csharp directory in Python.

Initial HowTo files with LobarPneumonia and ARDS are already present.

Add the correct ****_lung_affected values for the Exacerbation.

If process_requests recieves a "None" value, create a new
SEDataRequestManager object with a default set of requests for the
"Monitor" information.
This function should also serialize the data for processing.
It will require it to take a SerializationType as an additional
parameter.

Finish adding patient actions.  Remove "SEPatientAction" convenience
classes and and "HowTo" files for the new objects.

Added a HowTo that follows the Combat Multitrauma showcase scenario to show how to save and load an injured/disease state.

(its ok to get NaN from a Data Request)

Add the classes for specifying which data objects are passed back from
pull_data().

Update the HowTo_UseEngine to utilize these new classes.

Start adding the rest of the Patient Actions to the Python interface.

Add the __init__.py files which are needed for the setup.py to properly
seek and find the python files to install.

Add a hacky update to the GenerateCDMBindings to copy an instance
of the file into the two directories that are created as
the Python bindings are created.

Changed respiratory driver waveform similar to the equation previously used for smoother transitions.
Updated documentation.

Fixing a few bugs found in doing so

Add a setup.py file to capture and install the found source code for the
python subdirectory.

To use:

cd engine/src/python
python setup.py install

To Remove:
  Find site-packages and remove pulse-2.3.0***.egg/ directory.

TODO: Find proper method of including PyPulse****.pyd in installed code
to eliminate need to set PYTHONPATH.
