Even though some filters only make sense when using multiple pvserver
processes (e.g. Process Id Scalars filter) we want to make all of them available
so that Python tracing or Catalyst script generation can use them. Also
moved the XML definitions from proxies_mpi.xml to filters.xml for
filters that aren't directly using MPI.
