Run the `clang-format.bash` script to update all our C and C++ code to a
new style defined by `.clang-format`.  Use `clang-format` version 8.0.

* If you reached this commit for a line in `git blame`, re-run the blame
  operation starting at the parent of this commit to see older history
  for the content.

* See the parent commit for instructions to rebase a change across this
  style transition commit.

Includes a few changes related to extraneous whitespace fixes and
deduplication of headers as well.

This commit reindents the code with the following utility:
Utilities/Maintenance/vtk-reindent-code.py

This utility changes the positions of the braces so that they are no
longer indented relative to the code block they reside in.

The bash command line used was the following:

Include the module-wide header from every header in each module.
Reference the export/visibility macro for the module.

Move source files from their former monolithic VTK location to their new
location in modular VTK without modification.  This preserves enough
information for "git blame -M" and "git log --follow" to connect
modularized VTK files to their original location and history.

The CVS $Revision$ keyword replacement will no longer maintain these
macros automatically.  They were used only to implement CollectRevisions
and vtkObjectBase::PrintRevisions, an API that was never used.

ENH: Change decoded gradient table to be static (along with the method to compute it). Should make volume rendering tests faster

ENH: First check in of the vtkFixedPointVolumeRayCastMapper and related classes / tests. This mapper is stable, but is still a work in progress. Expect internal changes in the next few months.
